Recreational Soccer Forms

Recreational Soccer Signup Form - Fall 2010
Registration is now open for the Fall 2010 season. We have leagues for players from toddler age through 9th grade. Toddlers must be at least three years old as of August 1, 2010.

The season will start on September 25 and run through October 30. All games are on Saturdays, plus one weekday practice per week for all leagues except the toddler league.

You can mail in your registration to the address on the form, or leave it in our drop box in the lobby of Lyndhurst City Hall. We'll also have in-person registration on Saturday August 28, 2:00 to 4:00 pm, at Play It Again Sports, 5455 Mayfield Road. We have a special discounted fee this fall for those who register early. $40 per player before August 15, and $45 per player before September 3. After September 3 we'll accept late registration on a space-available basis, at a cost of $50 per player.
The registration deadline is September 3.

SELECT Travel Soccer Forms

SELECT Tryout Information/Registration Form
Fill out and sign the Player Information Form. The form must be completed for all new and returning players during spring tryouts. Tryout information for 2010-11.

OYSAN Player Registration Form
Fill out and sign the OYSAN (Ohio Youth Soccer Association North) Player Registration Form. This form is required by both the boys and girls leagues to register players with the league. The form must be completed for all new and returning players. When completing the form, the League Name should be "OASL" for boys and "NOGSL" for girls.

Multiplex Indoor Soccer Forms

Team Roster Form
The roster must list all players who will play on your team at the Multiplex during the indoor soccer season. The parent signature is no longer required on this form. One form per team.

Player Release Form
Each player on your team needs a Release and Waiver of Liability form. The form must be signed by the player's parent. One form per player.

Forms for Coaches

KidSafe Risk Management Disclosure Statement Form
All SELSC coaches, including recreational and SELECT coaches, must fill out and turn in an OYSAN KidSafe Risk Management Disclosure Statement every two years. The form is also required for assistant coaches.
Note: This is now an online form. When you fill out the form you must designate one or more leagues. League designation should be as follows: Rec league coaches specify "SELS". SELECT boys coaches specify "OASL". SELECT girls coaches specify "NOGS". Print a copy of your completed form before you click the "Submit" button, and turn the printed copy in to your club representative.

OYSAN Coach Registration Form
This form is required for all SELECT coaches and assistant coaches, when registering the coach with the league.

You can save the form on your computer by clicking on the form title with your right mouse button, choose "Save Target As...", and then indicate where the file should be saved on your computer.
